What We Do
We aim to provide support around implementing NICE guidelines around undernutrition. Providing best practice nutrition and hydration care involves five key principles (which incorporate NICE’s Nutrition support in adults quality standard (QS24) and clinical guidance (CG32)). These key principles are stated below, along with how our programme aims to support them:
2) Working together within and across organisations – we are involved in piloting and evaluating ways of establishing more integrated working across our region, and investigating the barriers to implementing integrated undernutrition care
3) Identifying malnutrition risk early using screening tools – we have provided training and produced training resources (e.g. presentations and case studies) on using the Malnutrition Universal Screening Tool (‘MUST’) to screen for undernutrition. We are also working with several organisations, including voluntary sectors, social services and care agencies to look at simpler ways of screening and identifying undernutrition risk in older people in the community
4) Developing individualised care plans – we developed local nutritional care pathways as part of the OPEN Eastleigh and Dorset Malnutrition Projects, and have since genericised these and included them as part of our OPEN toolkit. Our generic care pathways can be adapted to localise them for a specific area or locality. We work with groups including care homes, community nursing and practice nurses to encourage the use of individualised care plans to help reduce undernutrition risk
5) Monitoring and evaluating the impact of care on an individual’s outcome – along with our many projects which currently consider this, we have developed an evaluation framework as part of our OPEN toolkit.
