Mission Asthma, Mission COPD and NHSIQ Breathlessness all demonstrated significant reduction in key measures of healthcare utilisation. MISSION ABC will build on the successes of these three projects, embedding a robust model of care that will improve the lives of patients with respiratory disease, providing accurate diagnosis and medicines optimisation through guideline-based prescribing, patient and primary care education, enhanced self management and integrated follow-up.
An integrated approach to the management of complex patients has the potential to improve outcomes and reduce inequalities. Patient and health care professional feedback has highlighted a real need for such projects, addressing conditions that often are burdensome to patients and healthcare systems, and can be life-threatening.
Mission ABC will proactively identify and assess around 1000 patients in the SE Hants region, with the first clinics scheduled for September at Liphook Village and Liphook and Liss surgeries. The project is being co-funded with the generous support of the South Hampshire Multi-specialty Community Provider (MCP), which was one of the initial Vanguard sites chosen by NHS England to implement new models of care which will help move specialist care out of acute hospitals and into the community.
