Patient population: 140,000
The Isle of Wight covers a registered GP population of 140,000 and 17 GP practices. The Vanguard application team known as ‘My Full Life’ is a partnership consisting of Isle of Wight CCG, Isle of Wight NHS Trust, Isle of Wight Council and the GP collaborative One Wight Health.
The Vanguard is developing a person-centred, integrated healthcare service that is coordinated with social care. This will support better outcomes for people and through close working with local communities we will build capacity and resilience into the system. We will co-ordinate our use of digital technology through a single point of access that will also include patient-led monitoring to enable us to deliver care out of hospital and right across the island.
For example, a patient with a long-term condition, such as diabetes, will be supported, through monitoring and close working with their GP, to manage their condition to live the life they want to lead outside of hospital care.
