Multi-disciplinary and integrated care teams: Review of Insight evaluation reports 2016-2019

The focus of this report is to address four key questions provided by the WAHSN Healthy Ageing team in helping to support the development of a guidance on ageing for the purposes of setting up a GP led multi-disciplinary team (MDT) for frail older people.

Existing WAHSN Insight reports were used to conduct the review and provide an overview of the structure and function of integrated care teams, which are models of multi-disciplinary teams (MDTs). This review sought to understand whether these MDT structures were standardised and if there were key success factors particularly for information sharing. Also, how best to enhance MDT virtual working, and whether patients and carers were included in MDT discussions. 


This report is a synthesis of existing WAHSN Insight evaluations looking into setting up MDTs for frail older people and trying to identify key success factors
