Independent evaluation of the North East Hampshire and Farnham Vanguard Fleet Integrated Care Team

This report is one of five evaluating the locality Integrated Care Teams (ICTs) in North East Hampshire and Farnham. The five localities each formed their own ICT as part of the Vanguard programme in 2015 and these were evaluated separately during the latter part of the three-year programme.
