Independent Evaluation of North East Hampshire and Farnham Integrated Care Teams (ICTs)

This 2019 evaluation revisited the North East Hampshire & Farnham Integrated Care Teams, which had previously been evaluated during the latter part of the Vanguard programme. The evaluation found positive evidence of the continued development of the ICTs with sustained benefits for both patients and staff.
