This 2018 poster reports on a mixed-method evaluation underpinned by Normalisation Process theory and Force Field analysis using a structured focus group (including survey and brainstorming and ranking exercises) of staff to better understand enablers and barriers to embedding the ERS@H in a sustainable way on a long-term basis. This new care model involves a range of health and social care staff delivering holistic care and support to patients after earlier discharge from unplanned hospital admission.
