Primary Care Programme Manager

Sarah is one of our programme managers currently working within the Primary Care Programmes. She is going to be working with Wessex colleagues on the shape and co-design of local strategies for the development of primary care access, digital access and remote monitoring and supporting the uptake of innovation across the network. Sarah has joined us from the Hampshire, Southampton, and Isle of Wight CCG where she was a commissioning manager for planned care.

Prior to this she worked at the Isle of Wight Trust as a service manager for older persons mental health where she worked and led on the redesign of the dementia ward and implementing a community outreach model. Sarah has also worked across the country in a Primary Care Commissioning role as a national adviser focusing on Personal and Team Development in Primary Care teams and working with national projects including Care Navigation and the Clinical Pharmacist in Practice pilots’ phase one and two. Sarah started her NHS career in 2008 as a Commissioning Information Officer which led on to roles within the PCT as CCG Performance Analyst and then into Public Health as a Specialist Information Analyst supporting and writing JSNAs and Public Health Annual reports. Sarah is passionate about improving and using data and tools to support innovation and to enhance the delivery for primary care teams for both patients, clinicians and all practice staff.
