Wessex AHSN are pleased to have offered a series of evening sessions focusing on patient centred anticoagulation training. The aim of these sessions was to help GPs, Community Pharmacists and Practice Pharmacists to support patients prescribed anticoagulants and undertake patient centred discussions.
It is recognised that patients do not always absorb and understand key messages about anticoagulation when they are prescribed. GPs, Community Pharmacists and Practice Pharmacists are ideally placed to support patients and ensure their on going adherence to medication, prevent harm and contribute to improve patient care.
The evenings included information about consultations for anticoagulation and an overview of anticoagulation with some case study discussions. All the slide presentations and resources from these sessions are available on this page.

In 2019 we are developing videos to support understanding of some of the more complex aspects of Anticoagulation and AF management that we are finding from our project work in Practices.
