A range of resources to support health and care professionals in undertaking, assessing and supporting people living with frailty has been developed by the Wessex AHSN Healthy Ageing Acute Expert group.
Please see downloadable and online links in the resource section below. They have been reviewed by local academic peers from the University of Southampton and Bournemouth University.
Some of these resources can also be shared with patients, carers and relatives (Tier 1), these have been clearly annotated.
Work on finalising an Tier 1 e-learning package is in progress and will be made available in due course.
Leaflets providing information on communicating with people living with frailty, frailty screening and holistic approaches for assessing people living with frailty are now ready for download and use within your organisations.
Additional educational resources aimed at those with clinical leadership responsibilities (Tier 3) for people living with frailty are being developed by the British Geriatrics Society.
