Wessex AHSN Healthy Ageing frailty virtual ward project, aimed to complement work being scoped and developed nationally, regionally, locally and within other AHSN networks.

Working collaboratively with Wessex partners the project aimed to create a picture of frailty virtual ward provision across Wessex, understanding current terminology used. Comprising of three component parts, the project has helped to increase our understanding and highlight areas where innovation, sharing of best practice, and ways of integrating frailty virtual wards into existing services for those living with frailty, may benefit Wessex system partners.

The Wessex frailty virtual ward translator tool consists of a data capture tool supported by an implementation toolkit. A user-friendly, four-step approach has been designed to support integrated care system leads, and those working at local health and care service delivery levels in understanding the National NHSE frailty virtual ward definition, and creating service development plans.

Wessex AHSN Insight team completed a rapid insight evaluation of a Wessex FVW. Insights will help shape understanding of the practical implementation of standard operating procedures drawn up for a local frailty virtual ward.
