What was delivered in 2022-23? 

• We delivered or supported 13 educational sessions to a combined health and care audience of over 600 people.
• We produced a series of educational videos for healthcare professionals to support the education and management of CVD. 
• We supported GP federation Mid Hampshire Healthcare with the implementation of a new secondary prevention cardiovascular risk review pathway. The lifestyle clinics, which were delivered at 14 practices throughout Winchester and Andover, made recommendations about existing medications and therapies, or offered patients inclisiran, a new cholesterol-lowering injection recently approved by NICE after trials showed it can lower bad (LDL) cholesterol by around 50%. Our implementation report will support ongoing development of the pathway, which is one of the first federation-led delivery models in the country, and will inform other practices hoping to adopt a similar approach. By the end of March 2023, 53.8% of Primary Care Networks (PCNs) across Wessex had ordered inclisiran, benefiting patients across the region.

We worked with both Hampshire and Isle of Wight (HIOW) and Dorset Integrated Care Systems (ICSs) to support the identification and management of familial hypercholesterolaemia (FH), a genetic condition which prevents the liver from processing cholesterol properly. This has included:

• Supporting four PCNs in Dorset to successfully implement UCLPartners’ proactive care framework for blood pressure optimisation, which includes focus areas such as systematic risk stratification, health inequalities and workforce support. We can now take forward plans to build on this success, working to embed the framework within the remaining 14 PCNs in the region in 2023/24.
• Funding a HIOW pilot of FAMCAT 2, a tool developed by PRIMIS in collaboration with the academic research team at the University of Nottingham. The tool, which uses an algorithm to accurately identify patients with FH and high blood pressure in primary care, was used by HIOW Integrated Care Board’s (ICB) pharmacy team as part of their outreach programme.
